We are seeking a dedicated and community-driven professional to fill the role of Carer Support Coordinator. As the primary point of contact for carers in our network, you will be responsible for fostering meaningful relationships between carers and essential community resources.
Main Responsibilities
* To establish seamless connections between carers and vital community services, promoting holistic well-being and independence.
* Supporting carers to thrive, focusing on comprehensive care, going beyond basic needs to help carers achieve their full potential.
* To oversee the Carer Support Program, facilitating social interaction and promoting wellbeing, connection and a sense of belonging.
Key Requirements
* Experience working with family carers in mental health, alcohol and other drugs, community support services.
* Certificate IV or above in a support discipline and/or Equivalent Community sector experience.
* Current valid Driver's Licence.
* Strong technology skills.
* Satisfactory National Police Records Check (within 12 months).
* WWCC / QLD Blue Card.
* NDIS Workers Screening check / QLD Yellow Card.
* Appropriate IT Skills.
* Right to Work within Australia.
* NDIS Worker Orientation Module Certificate.
