The position of Community Geriatrics Team Leader is an exciting opportunity to lead a passionate multidisciplinary team delivering life-changing care to older adults in their own homes.
GEM@Home is a geriatrician-led multidisciplinary service, offering care to the older person, with complex medical, functional and or psychosocial needs, in their own home.
We are seeking a skilled leader who can provide services to community-dwelling patients via multiple modalities, including but not limited to face-to-face and Telehealth.
* Link clients and carers with relevant community resources and services
* Communicate effectively with the client's family, carers and other service providers
* Provide leadership to the Community Geriatrics team, including supporting recruitment, performance management and development, rostering, allocating tasks, and other human resource management
* Mentor and support other professional members of the team
To be successful in this role, you will need:
* Registered Nurse with at least 3 years, full-time equivalent, post-registration experience or an Allied Health professional holding an appropriate degree level qualification and membership (or eligibility for membership) with the relevant professional association, and or registration with the relevant Board
* Experience in supervising students, and less experienced staff
* Effective leadership skills, including highly developed skills in communication, problem solving, conflict resolution, and negotiation skills
* Clinical experience as a senior practitioner within a subacute setting, including clinical experience with complex patients
* Recent experience working in a specialist geriatric unit
This is a single vacancy, with two possible appointment classifications. The successful applicants will be appointed in the position appropriate to their skills, qualifications, and experience.
