You will have a chance to make a real difference to your local communities and the lives of those you work with.
About the Role
The Department of Justice is seeking a dedicated and experienced Chronic Disease Nurse Coordinator to join the health services team within the Western Australian Corrective Services system. This role focuses on improving the health and wellbeing of incarcerated individuals with chronic conditions such as diabetes, cardiovascular disease, respiratory illnesses, and other long-term conditions.
Key duties include clinical leadership in care coordination, oversight, comprehensive assessments, development and monitoring of individual care plans, and support for self-management through education and culturally appropriate health promotion. You will collaborate with prison health services staff, general practitioners, mental health and allied health teams, and custodial staff to ensure a patient-centred approach. You will contribute to discharge planning and continuity of care, including liaising with community health providers and Aboriginal health services where relevant. You will liaise with the local health department to place patients on appropriate registers to ensure continuity of care. The role also forms part of the policy review team and will contribute to developing policies and procedures related to Chronic Disease Management.
