About The Role

As our In‐Home Care Coordinator, you will play a key leadership role in coordinating and overseeing individualized services that support clients to live safely and independently at home. You'll use your Registered Nursing expertise to conduct clinical assessments, develop care plans, manage budgets, mentor team members, and ensure care is person‐centred, safe and high‐quality.

What You'll Do

* Conduct comprehensive assessments and reviews, and develop tailored Care Plans
* Provide ongoing case management in line with funding and clinical requirements
* Monitor client needs and adjust plans as circumstances change
* Lead, mentor and support In‐Home Care Carers and Domestic Assistants
* Collaborate with internal teams, GPs, allied health and external providers
* Support clients to navigate My Aged Care processes
* Participate in audits, quality improvement and accreditation activities
* Ensure person‐centred care aligned with wellness, dignity and cultural respect
* Provide clinical oversight consistent with Registered Nursing standards

Required Qualifications & Certificates

* Registered Nurse qualification & AHPRA registration
* Current CPR certificate
* Current QLD Driver Licence
* Safe, reliable and appropriately insured vehicle
* National Police Check
* Pre‐employment medical assessment

What You'll Bring

* Experience in care coordination, case management or similar roles
* Strong knowledge of aged care programs, Quality Standards, ageing and dementia
* Excellent communication, empathy and cultural awareness
* Skilled in care management / CRM systems
* High-level clinical judgement and understanding of chronic disease management
* Ability to work autonomously and collaboratively within a multidisciplinary team
