Job Summary:
This role is part of a dedicated team that focuses on providing holistic primary healthcare and community services. The successful candidate will work closely with patients to support their social emotional wellbeing, ensuring they have access to necessary resources and services.
Key Responsibilities:
* Support patients in attending appointments related to social emotional wellbeing
* Assist in coordinating administrative tasks for the SEWB team as required
* Conduct outreach activities as requested by the team
* Facilitate group activities to promote client engagement and support
* Liaise with clients to facilitate access to relevant services and support networks
* Maintain accurate reports and statistics associated with the role
Requirements:
The ideal candidate will possess a Certificate III in Health Worker Community Services or Clinical Practice. Mental health training experience is highly desirable.
