Clinical Nurse (THHS Single Assessment Organisation)

To provide clinical and non-clinical home-based face to face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool.
Under the Single Aged Care Assessment System, Aged Care Assessors work with clients and their representative/s to establish goals specific to the client's needs and develop appropriate strategies and actions, including referrals to aged care and non-aged care services.
The Townsville Hospital and Health Service Single Assessment Organisation is a multi-disciplinary team providing assessment services to older Queenslanders in the community and the hospital setting.
The assessment system aims to simplify and improve access to aged care services.
The assessment framework is designed to streamline the assessment process and enhance the overall experience for older Queenslanders.

Requirements
Qualification: Bachelor of Nursing or equivalent certification relevant to the position is mandatory.
Minimum of one year demonstrated experience directly delivering services in aged care settings and/or to aged persons.
This position requires the incumbent to operate a class C motor vehicle and an appropriate licence endorsement to operate this vehicle is required.
Travel: Please note, this position will be required to travel to, and work at, other facilities located within the Aged Care Planning Regions in the delivery of Aged Care Assessments.
This may involve day and/or overnight stays.
Travel on small planes or
This is a Vaccine Preventable Disease (VPD) risk role.
