Job Description
About the Role
As a Case Manager, you'll:
* Conduct comprehensive assessments to understand each client's unique needs.
* Collaborate with healthcare professionals and service providers to deliver tailored support.
* Build strong relationships with clients and key stakeholders.
* Ensure government funding is used effectively and in line with guidelines.
* Exceptional organisational skills and ability to self-manage.
* Strong communication skills – both written and verbal.
* Proficiency in using Microsoft Suite and client management systems.
* Commitment to working within a multidisciplinary team.
The Successful Applicant will have:
* Current Working with Vulnerable People/Children Check.
* Current National Police Check.
* Current Australian Driver's Licence.
