Senior Personal Carer | BlueCare | Community Care
Employment Status: Full Time - 12 Month Contract. Location: Gympie & Surrounding Areas. Remuneration: $37.77 p/h + 12% Super + Not-for-Profit Salary Packaging (increases take-home pay) + Allowances & Travel Reimbursement (KM) + Employee discounts.
Responsibilities
* Conduct initial client intake and screening assessments; develop and review individualised, client-centred care plans; provide quality care based on clients' needs.
* Conduct thorough initial screening assessments and develop care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
* Manage client information in CRM & CMS; keep details and case notes up to date.
* Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team.
* Spend time on the road completing scheduled visits; interact one-on-one with clients in their homes; support daily activities.
* Provide care in accordance with individualised Care Plans; support community-based recreational and social activities; assist with transportation, shopping and appointments.
Role requirements
* Respite Centre experience – highly regarded.
* Certificate III in Aged Care, Individual Support or equivalent – Essential.
* Current Australian Driver's Licence (Green Provisional minimum) – Essential. International licences will not be considered.
* Roadworthy & insured (Third Party Property minimum) vehicle to use for work – Essential.
* Experience with technology (smartphones, tablets, laptops, etc.).
* Confidence to satisfy probity checks required by legislation or policies.
