We also support those with mental health concerns and those transitioning from correctional centres.
Our Residential Aged Care Group Facilitator plays a vital role in enhancing the quality of life for older people through improved access to primary mental health services within a person-centred stepped care approach.
This program delivers in-reach healthcare services to residents in aged care facilities. Our services target residents with a diagnosed mental illness or those assessed as at risk of mental illness.
The facilitator will work collaboratively with staff to provide groups of 2-3 hours duration covering topics such as understanding mental health, reframing strategies, mindfulness and enhancing social connectedness.
Grounded in acceptance and commitment therapy, these groups aim to support residents in managing their mental wellbeing independently and improving their social, emotional and psychological health.
Responsibilities:
1. Confidently facilitating training and group information sessions to participants
2. Engaging with local providers, developing positive relationships and building a local presence
3. Collecting and administering outcome measures and providing feedback for continuous program improvement
4. Building rapport with participants and linking them to appropriate information and referral options where necessary
5. Being the face of our organization when visiting residential aged care facilities
The facilitator will be supported by an experienced Team Leader and Allied Health Practitioner - Social Worker in assessing referrals and receiving further support within our multidisciplinary team.
This is a highly rewarding role that would suit a new graduate or an individual with a nursing or nursing assistant background and relevant experience.
Requirements:
* Understanding of cognitive decline in the ageing population and its effect on overall wellbeing
* Ability to organise and deliver recreational activities and experiences to older persons alongside the ability to be creative and take initiative
* Excellent stakeholder management skills to enable effective working relationships with key stakeholders
* Proven effective engagement and communication skills with older persons
* Demonstrated ability to solve complex problems
* Ability to work collaboratively, cooperatively and effectively as part of a multi-disciplinary team
* Possess the essential qualities of compassion, empathy, active listening skills and maintain a strong work ethic
* Demonstrated understanding of mental health conditions and the mental health sector, especially regarding older persons' mental health
* Hold an Australian Driver's Licence
* Tertiary qualification (or working toward) in Social Work, Psychology, Social Science, Health or a related field is highly regarded
