Job Role: Extra Care Services Coordinator
The Extra Care Scheme Manager plays a pivotal role in delivering high-quality on-site care and support services within the scheme 24 hours a day.
You will be responsible for promoting the independence, security, and well-being of residents by working in collaboration with the on-site Housing Provider team as well as the Local Authority and third sector organizations involved in supporting the residents.
Main Responsibilities:
1. Oversee the overall operation of the scheme.
2. Ensure that robust, person-centered care plans are in place to enable staff to deliver agreed outcomes.
3. Maintain regular liaison with social workers, social services, commissioners, health authorities, District Nurses, voluntary organizations, and other professionals involved in the integration of the service.
4. Take charge of budget management, ensuring efficient allocation of resources.
5. Lead investigations into complaints, safeguarding concerns, or incidents, ensuring responses align with service level agreements (SLA).
6. Manage, support, and develop staff through care coaching, inductions, training, personal development reviews, and mentoring.
7. Produce, monitor, review, and report KPIs in line with company and local authority contractual requirements.
8. Attend allocation/panel meetings with the Housing Manager, make recommendations, and offer placements to potential residents, ensuring a balanced community of care and support needs.
9. Take responsibility for the Business Continuity Plan, working with the Housing Provider on Fire Evacuation and Emergency planning.
10. Collaborate with the Housing Provider to ensure the provision of a wide range of activities, encouraging participation.
11. Liaise with health services for administering, recording, and supervising medication according to policies and procedures.
12. Provide direct care and support to residents in emergencies where appropriate.
13. Work closely with relevant stakeholders, including professionals, care workers, families, and advocates involved in providing residents' care and support.
14. Attend supervision, staff, and team meetings, and training sessions as required, which may be off-site.
