About the Role
We're looking for a skilled and compassionate Care Coordinator to support a caseload of around 20–30 participants across our Supported Independent Living (SIL), In-Home Support, and Community Access Services.
In this pivotal role, you'll bring together your expertise in service delivery, leadership, and compliance to ensure participants receive the highest quality of care. You'll also play a key role in mentoring staff, monitoring service standards, and preparing for internal and external audits.
This is a part-time hybrid position, with three days in the office and two days working from home. We offer genuine workplace flexibility across our North Brisbane and Gold Coast offices, tailoring arrangements to support your lifestyle and professional goals.
The successful candidate will commence two weeks after the employment contract has been signed (or at a mutually agreed start date), allowing a smooth onboarding and integration into our dedicated team.
Key Responsibilities
* Coordinate participant supports and implement care plans across SIL and CAS programs.
* Supervise and support staff to ensure quality, compliant service delivery.
* Prepare for internal and external NDIS audits and maintain documentation.
* Manage rosters, shifts, and operational requirements.
* Liaise with families, clinicians, and support coordinators to ensure collaborative care.
* Monitor outcomes, risks, and key performance indicators.
* Contribute to quality improvement and compliance initiatives.
About You
* Degree in Social Work, Community Services, or related field.
* Strong knowledge of NDIS Practice Standards, restrictive practices, and PBS principles.
* Experience supporting participants with complex and high-intensity needs.
* Proven ability to manage staff, rosters, and day-to-day service operations.
* Confident communicator with excellent stakeholder engagement skills.
* Professional, ethical, and committed to continuous learning.
