About the Role
We are seeking a Clinical Coordinator to join our Middle Swan team. Reporting to the Facility Manager and working alongside another Clinical Coordinator, you will play a key role in delivering person-centred care, supporting clinical excellence, and ensuring compliance with regulatory and accreditation standards.
You will coordinate clinical operations, mentor and support staff, and foster an environment where residents' choices, preferences, and wellbeing are central to care delivery.
Key Responsibilities:
* Coordinate and oversee daily clinical care, ensuring care plans are personalised, resident-focused, and reflect current needs
* Plan, attend, and review Family Conferences to maintain strong relationships and respond to resident and family needs
* Support AN-ACC assessments and reviews, ensuring documentation is accurate and funding requirements are met
* Participate in continuous quality improvement initiatives and monthly quality committee meetings to support best-practice care aligned with Masonic Care WA's Philosophy of Care
* Identify staff education and development needs to maintain a skilled, competent workforce capable of delivering high-quality care
* Maintain clinical stock levels and ensure efficient use of medical supplies and resources
* Manage equipment and other resources to support staff in delivering effective, person-centred care
* Recommend improvements to the Quality Management System, collaborating with the Facility Manager and healthcare team to enhance resident outcomes
* Manage clinical incidents and feedback professionally, ensuring timely follow-up and continuous learning

* Registered Nurse (RN) qualification with clinical experience, ideally in Aged Care
* Proven leadership and team management skills, with the ability to influence, mentor, and motivate staff to achieve goals
* Demonstrated commitment to the delivery of person-centred care and resident wellbeing
* Knowledge of Aged Care Standards, accreditation, and AN-ACC requirements
* Experience in managing clinical documentation, quality improvement, and workforce development
