Job Description
* We are seeking a skilled Clinical Care Coordinator to join our multidisciplinary team in delivering integrated care and support for recovery outcomes.
* The successful candidate will conduct risk assessments, including assessment of suicide risk, violence/aggression and vulnerability, and provide risk management in collaboration with the individual, their family/carers and other support systems.
The ideal candidate will have:
* Full registration as an Occupational Therapist or Mental Health Nurse, General or Provisional registration as a Psychologist/Clinical Psychologist OR eligibility as a Social Worker.
* Knowledge & understanding of evidence-based interventions & clinical practice, as well as demonstrated excellence in the practical application of such, in mental health and alcohol use.
* Highly developed communication (including written) and interpersonal skills, with the ability to engage with people, advocate on their behalf, and establish effective relationships with staff, clients, families & friends and stakeholders.
* Experience in electronic client/customer management systems, desktop computer usage, and Microsoft Office applications.
* Eligibility to apply for NDIS worker screening clearance.
