Your opportunity: The Nurse Navigator Service is a state-wide model of care designed to provide holistic support to patients with complex health conditions requiring an expert level of healthcare co-ordination.
This integrated and proactive approach to health care service delivery will help ensure patients receive the most appropriate service when and where they need it.
Nurse Navigators work across system boundaries to actively advocate and improve health literacy, in close partnership with patients and families, other health and allied-health services to improve the patient's healthcare experience and quality of life.
The Nurse Navigator role employs advanced clinical and professional practice capabilities to deliver a positive patient centred health experience for individuals.
This role aims to facilitate a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating and empowering individuals and coordinating patient care.
The Nurse Navigator must demonstrate capabilities that support the effective coordination of patients with complex care needs across all speciality services, focussing on those with multiple co-morbidities and high frequency presenters for unscheduled care.

Our offer:
You will receive a competitive remuneration package value up to $54,017 p.a., comprising salary rates $66.86 - $72.59 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 7) (Permanent Part Time, 25 hours p.f.).
