Pathways to Community Living Initiative (PCLI) is a coordinated state-wide approach to supporting people with enduring and serious mental illness who have been in hospital for protracted hospital admission and/or are likely to have trouble with discharge solutions, to re-establish their lives in the community.
Illawarra Shoalhaven Local Health District is committed to building a professional workforce that supports consumers with a severe and persistent mental illness on their recovery journey. The Senior Clinician will work across multiple-sites providing evidence-based care that supports safe transitions of consumers into care in a community setting (or appropriate inpatient setting) based on the individual consumer's strengths, needs and wishes. These services are to be least restrictive, collaborative, with consumer and their nominated carers and family.
A primary goal of this position is

· To work collaboratively with staff, consumers with co-morbid serious and persistent mental health illness and identified intellectual disability or cognitive deficit, carers and other agencies (both within ISLHD and government and non-government organisations) to promote effective participation in mental health treatment, support and recovery.

· This role includes both direct specialist clinical work with clients and carers who are referred to the PCLI pathway; as well as inter-agency liaison work with local disability providers and community managed organisations to facilitate timely access to necessary supports.

· This role will have a focus on supporting people and their families understand and manage some of the complex issues that relate to intellectual disability and/or cognitive impairment.

Shellharbour Hospital is a busy acute care facility which provides emergency services, elective surgical services, general medicine, antenatal and primary family health care, satellite renal dialysis, and adolescent and adult mental health services on site. It's an accredited healthcare facility with the Australian Council on Health Care Standards (ACHS) and is recognised as providing a high standard of care to the community.

- Applicable vaccination requirements for a Category A role

- Working With Children Check (WWCC)

- Tertiary qualification in a health discipline (social work, nursing, occupational therapy, speech pathology or psychology) with current registration or eligible for membership with AASW

o Registered Nurses must hold relevant post-registration qualification and at least 3 years' experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate

- Valid NSW Drivers Licence
