About the Role:
The Carer Services Navigator will provide advice and guidance to carers on available services, identifying their needs and developing tailored support plans. This may involve coordinating financial and business functions to ensure seamless referrals and service delivery.
* Key Responsibilities:
* Provide expert advice and guidance to carers on available services and support options
* Develop and implement individualized support plans to meet the unique needs of each carer
* Coordinate referrals and service delivery to ensure timely and effective support
* Foster strong relationships with community partners and stakeholders to enhance service delivery and outcomes

Requirements:
