About the Role
We are seeking a Full time Transition Services Coordinator to lead a team supporting young people transitioning from out of home care to independent living.
In this role, you will lead and support Transition Mentors and specialist staff to deliver high-quality consistent case management. You will also maintain a caseload, providing direct support to young adults as they build independence and life skills.
This position plays a key role in driving service quality, modelling organisational values and achieving positive outcomes for young people.
The role is based in Sunshine Coast with regular travel to Gympie.
Responsibilities
* Lead and empower a team to achieve strong outcomes for young people transitioning to independence
* Oversee service delivery to ensure high-quality, consistent case management and support planning
* Maintain compliance through accurate documentation, reporting and adherence to service standards
* Build and strengthen partnerships with internal teams, government and community stakeholders
* Drive continuous improvement, staff capability and culturally safe, responsive practice
Qualifications, Credentials & Other Requirements
* Current Blue Card
* Licensed Care Service (LCS)
* Current Driver Licence
* Tertiary qualification in Human Services, Social Work, Psychology, Behavioural Science or similar
* Willingness to travel within the region
* Availability to work after hours and participate in an on-call roster

You are a confident and compassionate leader with a strong background in case management and a passion for supporting young people.
You will demonstrate:
* Proven experience leading and supervising teams
* Strong communication and stakeholder engagement skills
* Understanding of the needs of young people with care experience
* Experience in case management within social services
* Strong problem‐solving, decision‐making and crisis response skills
