Your Role as Support Facilitator
You'll deliver holistic non-clinical follow-up care, practical support and outreach to people directly after a suicide attempt by:
Ensuring individual and team practice is in accordance with the requirements of relevant legislation, including the Human Services Quality Framework, National Mental Health Standards and the National Recovery-Oriented Practice Framework
Providing direction to the team on psychosocial rehabilitation, social inclusion, and recovery orientated practice frameworks
Providing operational support and leadership to the team in the Program Manager's absence
Undertaking specific mentoring with team members where identified
Triaging referrals and provide consultation where necessary
In partnership with clients and key stakeholders, conducting thorough psychosocial assessments, develop, implement and review individual recovery plans through individualised goal planning, progress monitoring and regular reviews, which are flexible and responsive to the individual's needs and achievement of goals
Providing crisis support and safety planning as required
Providing assertive and practical support to access services related to individual needs. Outreach and in-home/ community-based support
Facilitating linkage to ongoing community supports as necessary
Ensuring client information is up to date and in accordance with support requirements, as necessary draft documentation and reports for a range of stakeholders
Undertaking professional development, which includes regular supervision and reflective practice
Identifying and assist in locating resources, services and supports that facilitate community integration, including social, educational, and vocational and leisure activities
Where appropriate, participating in developing and implementing quality and service improvement activities that continually improve the service we provide
This is a fixed-term opportunity until July ****
What We're Looking For
Tertiary qualifications in community, social welfare, health or a related discipline
Experience working in senior roles within a recovery orientated service framework for mental health and/or suicide prevention
Passionate about making a positive change in the lives of people managing a suicidality
Deep understanding of the principles of recovery in the mental health setting
Demonstrated experience working with people with complex issues associated with those experiencing suicidality
Capacity to work independently and collaboratively within a team environment including being accountable for the effective delivery of client services
Sound communication and interpersonal skills including the ability to engage, negotiate, advocate, collaborate and undertake joint innovative problem solving
A Valuable Addition, Yet Not a Must-Have
Experience providing care coordination within a mental health setting
Understanding of the principles and standards of the Disability Services Act (Qld ****) and the National Mental Health Standards and how they relate to providing effective and accountable services
Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)
Relevant probity checks required by legislation and WMQ Limited policy.
Current Driver's Licence and willingness to travel for work
