A highly skilled Care Partner is required to work within our organization, supporting older adults to live independently and well in their own homes.
**Job Description:** The role of the Care Partner works closely with clients, families, and multidisciplinary teams to develop and implement personalized care plans that align with each client's unique needs, preferences, and goals. This includes assessing individual client needs, coordinating care planning to ensure services are appropriate responsive person-focused managing delivery in-home support services ensuring compliance Aged Quality Standards building maintaining strong relationships providing ongoing support communication guidance throughout their care journey collaborating documenting reviewing ensuring compliance related activities engaging ongoing discussions facilitating case conferences as needed.
**Key Skills & Qualifications:
* Bachelor degree health-related discipline e.g Social Work Physiotherapy Nursing Certificate IV aged disability community qualifications preferred experience home care community required.
* Excellent communication relationship-building skills commitment advocacy person-centred approach knowledge Quality Standards requirements working collaboratively healthcare professionals carers external providers problem-solving organizational coordination skills operating multi-site environment current driver's license safe driving record willingness travel sites services excellent administrative computer skills ability work team empathy patience.
