ART (Acute Rehabilitation Team)– is a newly established, integrated rehabilitation service providing coordinated care across acute hospital settings, inpatient rehabilitation and the community. Designed to support patients across the full continuum of care, the service delivers timely, flexible and person‑centred rehabilitation, enabling safe transitions and improved functional outcomes.

The service is staffed by a multidisciplinary Allied Health workforce delivering comprehensive rehabilitation interventions for patients with complex and evolving needs. Team members apply strong rehabilitation skills and advanced clinical reasoning to support early rehabilitation, optimise function, and facilitate recovery across settings, including in patients' homes.

ART– operates with a strong focus on collaborative practice, clinical governance and shared decision making. Clinicians working within the service are expected to demonstrate sound professional judgement, adapt to changing service demands and work effectively both independently and within teams, and maintain a clear understanding of professional scope, accountability and safety. This will include opportunities for interdisciplinary practice to enhance outcomes and efficient use of clinical resources. ART‑ also incorporates virtual models of care, requiring team members to engage in telehealth, digital assessment and remote rehabilitation delivery as part of routine practice.

ART– represents a contemporary approach to rehabilitation—bringing together acute, inpatient and home‑based care within a single service model to improve continuity, efficiency and patient experience.

ISLHD operates four hospital sites in the Illawarra: Coledale District Hospital, Bulli District Hospital, Wollongong Hospital and Shellharbour Hospital.
