Overview
A key part of a new project in Glascote, Stonydelph and Belgrave areas of Tamworth, the Community Wellness Worker will work directly with individuals, families, and community groups to promote healthy lifestyles and reduce health inequalities. Working alongside patients, GP practice staff and staff and volunteers from other local organisations, you will help to identify needs, provide support, and connect people with resources to help with physical, mental, and social health challenges.

This role will be employed by, and based with, Heathview GP Medical Practice in Glascote Heath.
Working Hours: 28 hours per week
Contract type: Fixed term to end September 2026 initially. Subject to 3-month probationary period.
Job responsibilities
* Key Duties
* Work with an identified group of individuals and their households to provide personalised support to address the causes of their poor health and wellbeing and identify what matters to them.
* Take a holistic whole-person approach in relation to physical, mental, emotional and social health and wellbeing and resilience.
* Use a range of methods to develop our patients knowledge, skills and confidence to tackle their own problems and challenges affecting their health and wellbeing.
* Work alongside people as equal partners, actively listen to what matters to them, build trust and rapport to help them recognise what they need, and connect them with the best available support or intervention to meet their need.
* Help people to identify barriers preventing them from accessing local resources or existing services appropriately.
* Identify health and well-being needs through various methods, including home visits, community outreach, and data analysis.
* Understand the local and accessible services and resources available and signpost to services in the community to support peoples/families health and wellbeing needs.
* Identify gaps in available services and resources preventing individuals and communities from achieving optimal health and wellbeing.
* Build relationships with local organisations and groups.
* Manage data and information and maintain accurate records of client interactions, progress, and referrals.
* In daily work, interact with: individuals and families on a one-to-one basis and in groups; community-based organisations and service providers (including voluntary or charity-based providers); NHS and local authority health and care professionals; lay and professional workers from other sectors; peers (paid and voluntary); other workers (paid and voluntary) who they may supervise; local health and wellbeing services (e.g. lifestyle support services, IAPT); sources of digital help and support; and the private sector (e.g. retail firms, local businesses).
Person Specification
Knowledge and Aptitude
* Understand the local health and social care system and other available community resources.
* Understanding of confidentiality, safeguarding and data protection.
* Approachable and friendly manner
* Demonstrates empathy and compassion
* Demonstrates understanding and care for individuals facing health challenges.
* Full driving licence and use of a vehicle for business purposes
* Knowledge of local area of Glascote, Stonydelph and Belgrave
* Demonstrate innovative ways of working
Qualifications
* Good standard of English and Maths
* Care Certificate
Experience
* Previous experience of a patient facing role dealing with individuals and families
* Previous experience of managing a caseload and delivering outreach services (e.g. home visits, groupwork etc)
* Experience of problem solving, identifying solutions to complex issues and connecting individuals with appropriate resources.
* Work in a health and social care role
Skills
* Able to manage a caseload, prioritise tasks, and maintain accurate records.
* Able to work autonomously within the scope of the role and within legal and ethical boundaries
* Able to work within teams and experience of liaising with colleagues at all levels
* Skilled in daily use of Microsoft Office software and use of patient/case record systems
* Able to produce reports using data
* Demonstrate active listening skills and communicate effectively with diverse individuals and groups.
* Use of EMIS(Optum) GP clinical system
