The Role
We have an exciting and fast-paced opportunity to join our Klemzig SA Clinical Care team as a Clinical Care Coordinator & Nurse Mentor. This full-time role reports to the General Manager, with support from the Clinical Care Manager, and involves supervising and supporting our passionate care team. The key purpose is to ensure residents at our Klemzig SA Care Home receive the highest quality clinical care. The Clinical Care Coordinator & Nurse Mentor manages care needs, ensures documentation and assessments are current, and aligns practices with best-in-class clinical standards.
Key Responsibilities
Provide clinical expertise and lead the clinical and care service teams across the facility.
Assist the Clinical Care Manager in leading care teams to meet all clinical needs of residents, including monitoring clinical indicators for best practice outcomes.
Manage assessment, documentation, and submission of AN-ACC requirements within the facility.
Support documentation, policy, procedure, and IT system compliance and usability within the clinical team.
Coordinate assessment, care planning, and review processes.
Support the admission process for new residents, including assessments and initial consultations with residents and families.
Ensure accurate record-keeping in accordance with organizational and legislative requirements (AN-ACC, resident records, staff time recording).
Contribute to staffing rosters, ensuring optimal skill mix for resident care.
Manage and document clinical risks and pursue continuous improvement initiatives.
You will offer
Innate empathy for residents and a commitment to improving clinical care quality.
Minimum 1 year experience as a Clinical Care Coordinator or similar leadership role.
Availability to work Monday to Friday, 2pm to 10pm.
Current unrestricted Registered Nurse Div 1 registration with AHPRA.
Postgraduate qualifications in aged care are advantageous but not essential.
Extensive knowledge of aged care clinical assessments, care plans, AN-ACC, accreditation compliance, and documentation.
Engaging leadership style capable of influencing and developing clinical teams.
High computer literacy to maintain documentation and reporting standards.
Excellent communication and interpersonal skills, with a passion for leading clinical outcomes.
Ability to manage multiple stakeholders with a balanced, calm approach in stressful situations.
Valid NDIS Workers Screening Clearance or ability to obtain one.
