What You'll Be Doing
Providing advanced and complex patient care within the Integrated Chronic Disease Management Community Health that reflect current standards of nursing practices and Mid North Coast Local Health District policies and procedures.
This exciting permanent position will provide a mix of Hospital Consultation Liaison (HCL), ambulatory withdrawal management and Outpatient Clinic across the Coffs Clinical Network region as part of a senior clinical nursing team.
About You
You are a registered nurse with current registration with the Nursing and Midwifery Board of Australia. You bring demonstrated expertise in the assessment and management of chronic disease management and thrive working both independently and as part of a multidisciplinary team. You are skilled in mentoring and coaching nursing staff, leading initiatives that enhance service delivery and patient outcomes, and communicating effectively using a range of written, verbal and digital tools.
