Description Your duties and responsibilities will include: delivering direct and comprehensive dialysis nursing care and individual case management to patients or clients within a home setting; being responsible for planning and coordinating services, including those of other disciplines or agencies, to individual patients or clients within a home setting; providing education, counselling and group work services orientated towards the promotion of health status improvement of patients and clients within a home setting; providing support, direction and education to newer or less experienced staff; participating in or assisting in the management of action research projects, and participating in quality assurance programs and policy development within a home setting. Skills And Experiences Applicants must have: Experience using Fresenius (4008/5008) and/or Gambro machine. On-the-job training is available to up-skill staff; Bachelor level Degree in nursing and a minimum of 2 years of relevant professional experience; Recent clinical experience in haemodialysis in a hospital/clinical environment; Current registration as a Registered Nurse with AHPRA; Current Working With Children Check (WWCC) and Criminal Record Check clearance certificates; Updated vaccination/immunisation record; Australian working rights; Demonstrated ability to plan and coordinate patient care & competency in clinical problem solving; Understanding & demonstrated participation of quality improvement processes/initiatives. Demonstrated competency in written and verbal communication and ability to maintain accurate clinical and assessment data. Current ALS and BLS certification essential.
