The Role

We're looking for someone who is passionate about youth mental health to fill the vacant position of Case Manager. The position sits within the headspace Early Psychosis Continuing Care Team based at our Joondalup Office. As part of the multidisciplinary team, the Case Manager will provide comprehensive case management and psychological therapy to young people aged 12 – 25 years, as well as the provision of crisis intervention and community treatment to young people within the service.

Case Managers will be required to work standard office hours with the occasional late night until 7pm.

The successful candidate will

* Provide case management within a community based multidisciplinary team (allied health, nursing, and psychiatry).
* Assess and treat 12–25-year-olds experiencing moderate to severe symptoms of psychosis.
* Provide clinical interventions with a focus on "least restrictive practice" and upholding principles of the Mental Health Act 2014 (WA).
* Ensure seamless care delivery, inclusive of families, other community mental health services, community-based psychologists, schools, and inpatient settings.
* Provide care coordination (psychosocial, psychiatry appointments, metabolic and other physical health screening), individualised clinical treatments and family support.
* Develop, implement and evaluate appropriate treatment plans for each young person and their family in collaboration with young people, families and members of the multidisciplinary team.
* Provide crisis interventions and manage risk within established clinical framework.
* Possess the relevant tertiary qualification (Registered Nurse, Occupational Therapist, Social Worker, Registered Psychologist) and be eligible for membership with the appropriate professional body (AHPRA or AASW).
