Job Description
This is a rewarding leadership opportunity where you will play a pivotal role in shaping the health outcomes of remote Aboriginal communities. You will be part of a supportive collaborative team working to make a lasting impact on the health and well-being of vulnerable populations. The Chronic Care Manager Lead also oversees regional implementation of the Territory Kidney Care (TKC) initiative in partnership with Menzies School of Health Research and NT Health. This includes integrating data-informed renal care planning and risk-reduction strategies into Sunrise's PHC model to address the high prevalence of kidney disease in remote communities.
Key Responsibilities:
* Lead and oversee chronic disease programs and multidisciplinary staff.
* Ensure evidence-based care planning and quality improvement.
* Coordinate specialist outreach and partnerships.
* Manage data reporting and compliance requirements.
* Support recruitment performance and clinical development.
* Represent Sunrise in regional governance and advisory forums.
Required Skills and Qualifications
This position is ideal for an experienced clinician and manager with strong expertise in chronic condition care and a commitment to improving health outcomes for Aboriginal people through coordinated culturally appropriate services. To succeed in this role you will need:
* Minimum 5 years in chronic disease management and/or regional health program delivery.
* Clinical supervisory or leadership experience in a remote Aboriginal health context.
* Demonstrated understanding of chronic conditions and their social determinants.
* AHPRA registration as Registered Nurse Allied Health Professional or equivalent.
* Non-adverse National Criminal History Check.
* Current OCHRE clearance.
* Acceptable vaccination status to work with vulnerable people in the NT.
* Right to work in Australia.
