* Key Responsibilities:
* Develop comprehensive health plans, ensuring the coordination of multidisciplinary services as required.
* Nurture strong relationships with customers, guiding them through their care journey and empowering them to achieve their goals at home.
* Maintain accurate records using enterprise systems and contribute to quality improvement initiatives.
Requirements for Success
* Prior experience in community or aged care, including case management and coordinating multifaceted care within the home setting.
* Strong customer focus, excellent verbal and written communication skills, and ability to engage in sensitive conversations with empathy.
* Knowledge of relevant funding streams, quality standards, audits, and experience supporting care transitions such as discharge planning.
* A collaborative mindset, capable of working effectively within a team-based environment, liaising with families, clinicians, and external providers.
