Key Responsibilities:
Develop and implement care plans aligned with client needs and aspirations.
Conduct assessments and create person-centered care plans focusing on client independence, wellness, and reablement.
Assist clients and their families in understanding Home Care Package funding guidelines to achieve positive outcomes.
BUILD effective partnerships with stakeholders, including clinical experts and allied health professionals.
Foster long-lasting relationships with clients and their families through empathetic and responsive approaches.
About You
We seek a compassionate and reliable individual with customer service excellence, ideally with experience in health and/or aged care sectors.
You will have attention to detail, a positive attitude, and well-developed skills in person-centered care principles.
A current driver's license and diploma-level qualifications in health/aged care/disability/social welfare are preferred.
