Role Description
This is a contract, on-site role for a Social Worker located in Melbourne, VIC. The Social Worker will provide support to clients by assessing their needs, developing personalized care plans, and facilitating access to resources and services. Key responsibilities include advocating for clients' best interests, collaborating with caregivers and other healthcare professionals, and monitoring progress for improved outcomes. The role requires maintaining proper documentation and adhering to ethical standards and organizational policies.
Qualifications
* Strong interpersonal, emotional intelligence, and active listening skills
* Experience in case management, care planning, and conducting assessments
* Knowledge of social work practices, community resources, and disability or aged care regulations
* Competence in documentation, report writing, and maintaining client confidentiality
* Effective communication and problem-solving abilities
* Relevant qualifications in Social Work or related field (e.g., Bachelor's or Master's degree)
* Current AASW member registration
* Ability to work independently and collaboratively within a team
* Prior experience in disability services is an advantage
* Understanding of cultural sensitivities and ability to work across diverse communities is preferred
