About the Role

">

This position plays a vital role in improving care coordination and reducing unnecessary hospital visits for Aboriginal and Torres Strait Islander people living with chronic and complex conditions.

">
">
* Advocate for Aboriginal patients across acute and emergency settings.
">
* Collaborate with clinicians to promote culturally appropriate care and education.
">
* Support clients and families in developing and engaging with goal-oriented, person-centred care plans.
">
* Foster partnerships between service providers.
">
* Provide tailored health education and encourage active participation in self-management programs.
">
* Assist with capacity building by participating in education, training, and quality improvement initiatives.
">
">

Key Responsibilities:

">
">
* To support clients by promoting early intervention, self-management, and culturally safe care pathways.
">
* Collaborate with hospitals, community health, aged care, and Aboriginal Community Controlled Health Organisations (ACCHOs).
">
* To foster partnerships between SWH, ACCHOs, and other service providers.
">
* Provide tailored health education and encourage active participation in self-management programs.
">
">

Requirements:

">
">
* Relevant qualifications in Nursing, Allied Health, or Aboriginal Health, with current AHPRA registration (as applicable).
">
* Strong skills in assessment, care planning, and service delivery.
">
* Proven ability to work independently and within a collaborative team environment.
">
* Excellent communication skills in high-pressure, multidisciplinary settings.
">
* In-depth understanding of community-based supports and networks.
">
* A minimum of 5 years' experience in an acute or community health setting.
">
* Advanced skills in time management, planning, and prioritising tasks.
">
