About the Role
St Vincent's Private Hospital Melbourne (SVPHM) is looking for a Social Worker to join our Continuum of Care Department. The position is based across Fitzroy, East Melbourne, Kew and Werribee campuses and is a Grade3 Social Worker role comprising three days per week. The Social Worker will provide support to the post‐discharge care team and help discharge patients with complex social needs.
Responsibilities
* Provide support to the post‐discharge care team across multiple campuses.
* Work autonomously and within multidisciplinary teams to support the discharge of patients with complex social needs.
Qualifications
* Minimum qualification of a Bachelor of Social Work.
* Five or more years of experience working as a Social Worker in a hospital environment.
* In‐depth knowledge of the aged care and community services sector.
* Strong understanding of the diverse issues affecting Aboriginal & Torres Strait Islander peoples.
Requirements
* Criminal record and/or child protection checks will be conducted and will form part of the selection criteria for this role.
* Compliance with the SVHA Staff Health Screening Immunisation Policy, including mandatory immunisations such as flu vaccinations and, where required, COVID‐19 vaccination.
