Our priorities include cultural awareness and safety, ongoing professional development, and quality improvement through evidence-based practices.We are seeking a proactive Social Worker/Specialist Support Coordinator to join our enthusiastic team.The RoleThis role involves working with clients with complex care and medical needs across their lifespan, identifying and addressing barriers to implement their NDIS plans. The successful candidate will build relationships with clients, families, disability service providers, clinical staff, and government departments, including health, education, communities, and justice sectors.Eunoia Lane promotes a collaborative, person-centered approach to support delivery, focusing on building a foundation for best practice care, staff training, and sustainable outcomes following crises.Key RequirementsA bachelor’s degree in social work or a relevant discipline (Occupational Therapy, Physiotherapy, Psychology, Nursing, or other Allied Health).Strong oral, written, and interpersonal communication skills.Ability to work independently and as part of a team.Active listening skills and empathy.Excellent organizational skills, with the ability to manage time and prioritize tasks effectively.Current Working With Children card, NDIS Worker Screening, Police check (within 6 months), valid driver’s license, and a roadworthy vehicle.Proficiency in Microsoft Office.Experience working with clients with dual diagnoses; experience in Forensic Disability or Forensic Mental Health is desirable but not essential.Willingness and ability to travel by car, 4WD, or light aircraft to rural and remote areas.Experience working with Aboriginal and Torres Strait Islander peoples and an interest in indigenous advancement.Experience supporting Culturally and Linguistically Diverse (CALD) communities and their networks.Eunoia Lane Culture and BenefitsWe prioritize professional development, staff support, and lifelong learning.
