Job Summary
We are seeking a skilled and compassionate Care Navigator to provide high-quality clinical assessments, advice, and referrals to clients in their homes.
About This Role
In this key position, you will work closely with local allied health professionals to facilitate responsive care in clients' homes. You will also lead a team of Care Advisors and Home Care staff, providing health education and promotion, including social support groups and carer education.
Key Responsibilities
* Conduct thorough clinical assessments, reviews, and care needs for clients in their homes.
* Provide expert clinical advice and support to clients, referring them to services where more specialized support is required.
* Lead a team of Care Advisors and Home Care staff to achieve excellent outcomes.
* Collaborate with local allied health professionals to deliver seamless care in clients' homes.
Requirements
* Bachelor of Nursing or equivalent qualification with current AHPRA Div 1 Nursing registration.
* Minimum 3-5 years experience as a Registered Nurse in Community, Commonwealth Home Support or District Nursing.
* Strong communication and interpersonal skills, with a positive outlook and a passion for person-centered care.
