Salary: $85,664 (FTE) per annum plus 12% superannuation depending on experience

Description
About the role
The Case Support Worker role supports case coordinators to deliver client-focused activities aligned with individual goal plans, assisting clients to build independent living skills, access community services, attend appointments, and sustain their tenancies.
Key responsibilities:
* Contribute to a high-performing, collaborative team culture built on respect and trust
* In consultation with case coordinators, deliver flexible, person-centred health, social, creative, and skills-based activities that support independent community living
* Work collaboratively to deliver outcomes aligned with HCA values, priorities, and policies
* Build and maintain strong partnerships with relevant service providers and agencies
* Ensure client wellbeing and safety are central to all referrals and activities
* Collect and analyse program data to identify trends and inform service delivery
* Identify and manage risks in collaboration with the Supportive Housing Team Leader
* Deliver innovative, client-focused services that reduce the risk of returning to homelessness
* Participate in supervision, reflective practice, performance planning, and monthly KPI reporting
Skills And Experiences
About you
You will bring:
* Relevant tertiary qualification in Social Work, Mental Health, Counselling, Social Sciences, or Certificate IV in Community Services
* Experience working in a similar role with comparable client groups
* Knowledge of homelessness-related issues, including mental health, alcohol and other drug use, and comorbidity
* Understanding of the South Australian human services network and outreach service delivery
* Experience supporting people toward independence, recovery, and reduced reliance on social services
* Compassionate, non-judgemental, and collaborative approach, with strong engagement and coaching skills
