About this Community Healthcare Role
We are seeking an experienced Registered Nurse to join our team and work in a community-based setting. This role offers the opportunity to provide high-quality, person-centred care to individuals with disabilities, working collaboratively with a supportive team.
This position is ideal for nurses who want to apply their skills and knowledge in a flexible and autonomous environment. You will be responsible for supporting the caseloads of the clinical nurse team across the Perth metropolitan area, providing hands-on nursing care, completing care plans, and training support workers as required.
Key Responsibilities:
* Provision of in-home nursing care, including catheter, PEG, and wound care
* Completing care plans as delegated by the clinical nursing team
* Providing training to support workers as required
* Other tasks within the scope of practice as delegated by the clinical nurse team
This role also offers opportunities for professional growth and development, with the potential to act up and gain leadership experience.
About You
We are looking for someone with excellent interpersonal and communication skills, demonstrated by building long-lasting relationships with customers, their families, teams, and external stakeholders. You should also have strong clinical knowledge and documentation skills, a collaborative mindset, and excellent stakeholder skills.
Essential Requirements:
* Current AHPRA registration in the category of Registered Nurse
* NDIS Worker Screening Check
* NDIS Workers Orientation Module
* National Police Clearance (no older than 6 months)
* Current First Aid Certificate (HLTAID010 or HLTAID003 minimum)
* COVID-19 Vaccination Certificate
* Current Driver's License
* Full Australian Working Rights
