THE ROLE
Reporting to the Practice Manager, the Registered Nurse is responsible for providing exceptional hands on care to patients from within their community, conducting health assessments to assist with the early detection of chronic disease, preparing management plans with patients on behalf of GPs, providing coaching for patients on health change principals and providing suitable action plans to reach specified goals and management of chronic disease. This role would be suited to those who have come from a hospital setting within CDM/Diabetes/Dialysis.
THE ESSENTIALS
- Full AHPRA registration without notation
- Full, unrestricted and ongoing working rights within Australia
- Previous experience within a General Practice or Chronic Disease Management setting
- An understanding of KPIs and how to implement improvements in practice
- The ability to support and mentor the learning of junior/graduate nurses or those new to practice nursing
- Excellent clinical skill and a respectful, professional bedside manner
- Strong communication skills to nurture relationships with patients, colleagues and GPs.
