Key Responsibilities
* Scheduling and Facilitation: Coordinate and facilitate support services to community-based clients, ensuring seamless delivery of care.
* Partnership Development: Foster and maintain strong partnerships with Disability Support Workers, promoting effective collaboration and communication.
* Documentation and Record-Keeping: Maintain accurate and confidential documentation, recording client care preferences and relevant service delivery information.
* Issue Escalation: Identify and escalate issues related to service delivery and client care to clinical and/or management bodies when necessary.
Requirements
* Previous Experience: Previous experience in a comparable scheduling position, preferably in the Disability sector.
* Skills and Qualifications: Excellent interpersonal and communication skills, with the ability to work collaboratively with various stakeholders.
* Driver's License: Current unrestricted Driver's License.
* Working with Children's Check: DHS Working with Children's Check (mandatory).
