Home Care Coordinator Opportunity
We are seeking a dedicated and passionate individual to fill the role of Home Care Coordinator. In this position, you will be the key liaison between our clients, their families, and our care team.
About the Role:
As a Care Coordinator, your primary responsibility will be to ensure that our clients receive exceptional home care services by managing schedules, coordinating care plans, and providing empathetic support.
Key Responsibilities:
1. Match clients with caregivers based on needs and preferences
2. Schedule and coordinate in-home care services
3. Communicate with families, caregivers, and healthcare professionals
4. Monitor and update client care plans
5. Ensure compliance with regulatory standards and best practices
6. Provide outstanding customer service and client support
Required Skills and Qualifications:
1. 2 years Experience in aged care/disability care and knowledge of relevant regulations preferred
2. Strong organizational and communication skills
3. Ability to multitask and work in a fast-paced environment
4. A caring and compassionate attitude
5. Proficiency with scheduling software and basic computer skills
Certificate III/IV in Individual Support (Ageing and Disability) or equivalent required.
