Conduct strengths-based assessments and create personalised care plans aligned with client needs and contractual obligations Provide person-centred case management and coordinate services for clients and carers with moderate to complex needs Manage budgets and ensure effective resource use Collaborate with health professionals and community organisations Maintain accurate records and meet reporting requirements Adapt to challenges with resilience and a solutions-focused mindset About You
You're an experienced professional in aged care (perhaps a Home Care Package Coordinator, Care Partner, Enrolled Nurse, or CHSP Coordinator) ready to take the next step in your career. You bring: Certificate IV in Community Services (or equivalent) and a current First Aid Proven experience in case management, service coordination, and care planning Strong understanding of SAH budgets and community care compliance Excellent communication, decision-making, and time management skills Confidence using digital tools (Word, Excel, calendars) Advantages
