* Our technology platform empowers older adults to self-manage their service providers with assistance from our clinical and customer support teams.
(Role Responsibilities)
1. Effectively deliver on all requirements of the Clinical Roster including:
* Monitor and triage the Clinical Inbox and wellbeing notes.
* Complete clinical assessments and reviews (e.g., VATs, CNAs).
* Support customers and families with informed decision-making.
* Guide palliative care and advance care planning.
* Respond to incidents and clinical escalations.
* Contribute to continuous improvement and clinical quality initiatives.
* Provide clinical input across our broader team and provider network.
* Align your practice with the Aged Care Act, Quality Standards, and our Clinical Governance Framework.
Required Skills and Qualifications
* AHPRA Registration as a Registered Nurse.
* First Aid certification.
* Experience in Home and Community Care, ideally with aged care clients.
* Strong clinical decision-making and case management skills.
* A warm, human approach and genuine passion for empowering others.
* Excellent communication and customer service skills.
* Bonus points if you know your way around self-managed or consumer-directed care.
