In collaboration with funders, we develop cutting-edge programs powered by clinical expertise and technology, all designed to relieve health system pressures and enable greater access to healthcare.About this opportunityAs a Care Integration Nurse (or Paramedic), you will collaborate as part of a broader team working closely with Medical Officers, Nurse Care Coordinators, our service provider network, and the Local Health Networks (LHNs).You will be responsible for delivering patient-centred care to suitable patients for MHH and undertaking clinical reviews as part of a multi-disciplinary team.MHH is committed to providing safe, high-quality, and effective health care services that address the holistic (physical, social, psychological, and spiritual) needs of our patients and their families and carers.
Our design of the MHH model has been informed by core principles that embody the change that we want to see (and create) in acute care.This role will involve undertaking visits in the community in our patients' homes delivering nursing care as required by their care plan.
You will have use of our company vehicle for patient visits starting your shift from our office in Eastwood.This position will be offered as either casual or part-time.
You will be rostered across a 7-day roster with two shifts: 7am to 3pm or 2pm to 10pm.About YouYou will be naturally empathetic, observant, attentive, trustworthy, clinically sound, and committed to making My Home Hospital a centre of excellence.Working in a dynamic environment applying your clinical skills, you will think on your feet and problem solve to ensure our consumers are provided with best practice care.You will also have :Bachelor of Nursing or Bachelor of Paramedicine & current unrestricted AHPRA registrationCurrent Basic Life Support certificationWorking with Children Check (WWCC) or willingness to obtainClinical experience in the management of acute medical and surgical patients, intravenous cannulation skills, or willingness to achieve this competency; experience with comprehensive clinical assessments will be highly regarded.Understanding of National Safety and Quality Health Service StandardsAnnual leave and personal leave entitlements (pro-rata if part-time)Use of MHH company vehicle with fuel card includedCollaborative team culture delivering this innovative programOpportunity to leverage internal career opportunities across the Medibank businessWhat's next?If your skills and experience match, we will be in contact shortly.
