As an Aftercare and Suicide Prevention Coordinator, you will provide empathetic support to individuals experiencing suicidal ideation.
This role involves working with a leading support services provider in Western Australia, focusing on case management and outreach within the Wheatbelt region.
You will work closely with clients, their families, and healthcare professionals to ensure comprehensive care and support.
Key aspects of this position include:
1. Case Management: Develop and implement personalized plans for clients requiring ongoing support.
2. Outreach Services: Provide regular check-ins and support to clients in remote or underserved areas.
3. Collaboration: Work closely with healthcare teams, including doctors, nurses, and social workers, to ensure seamless care transitions.
