Role
Social Worker Grade 2 – Infectious Diseases Unit/HIV Service Outward Program.
This is a permanent, full-time position within the HIV Outward Program.
The Social Worker provides social work services to patients living with HIV and their families/significant others as part of The Alfred Statewide HIV Service.
The role works across inpatient, outpatient, and community settings and may include inpatient social work for general Infectious Diseases patients.
Responsibilities include supporting HIV social work outreach clinics and collaborating with the HIV Outward Team, Nursing, Medical and Mental Health Clinicians.
The position involves a component of community outreach and discharge planning to ensure continuity of care.
Key Responsibilities
Provide comprehensive psychosocial assessments and reviews and facilitate timely, quality care plans.
Offer counselling to individuals newly diagnosed with HIV, those adjusting to illness, or facing changes in care needs, including addressing HIV stigma.
Collaborate with a multidisciplinary team to support patient care and discharge planning.
Develop and maintain community linkages and partnerships, especially in the HIV sector.
Consult with HIV Social Work Team Leader, HIV Senior Social Worker, and other disciplines as needed.
Inpatient social work to HIV inpatients to ensure continuity of care when required.
Qualifications and Skills
Advanced social work skills with preferably 3+ years post-qualifying experience and a commitment to ongoing professional development.
Experience in health settings with an understanding of the issues facing diverse patient populations and hospital admission/discharge planning.
Ability to work in fast-paced acute or outpatient hospital environments and crisis-responsive community contexts.
Understanding of HIV stigma, LGBTIQ, loneliness/isolation, CALD/First Nations populations, homelessness, family violence, financial vulnerability, NDIS, and aged care considerations.
Ability to provide counselling related to illness adjustment and supervise staff; interest in professional supervision of others.
Ability to lead a portfolio, clinic, or program within the team.
Good working knowledge of hospital and community settings and electronic medical record systems.
