We are seeking a dedicated and compassionate Case Manager to join our team, providing case management support to individuals who are homeless or at risk of homelessness.
As a Case Manager, you will work closely with participants to develop, action, and review individualized case management plans, taking into account their unique needs and circumstances. This may involve providing practical and personal support, crisis budgeting, information, advice, and community referrals.
You will also conduct initial and ongoing assessments of participant needs, including risk assessments and safety planning as required. Additionally, you will provide services that promote independent living, empower participants, and advocate for their access to housing and specialist services such as drug or alcohol treatment, mental health services, legal assistance, domestic violence support, and family violence services.
This role requires a strong ability to build and maintain relationships with various stakeholders, identify and embed solutions, manage time effectively, and communicate complex information in a clear and concise manner.
We encourage applicants with relevant experience in community services or a related field to apply. A Certificate IV qualification in Community Services or similar is highly regarded. Mandatory Requirements:

* 2 years relevant experience
