Make a Difference Every Day…
As a valued member of our Community Liaison team, you will:
* Oversee and coordinate the efficient discharge of patients, ensuring continuity of care and safety
* Monitor and facilitate patient flow across departments to optimise bed availability
* Apply clinical reasoning to assess and coordinate complex patient care needs, post-discharge
* Build collaborative relationships with patients, families, GPs, community services, and internal teams
* Navigate healthcare resources and contribute to efficient business operations and care outcomes
What makes a difference for us?
We're seeking an individual who brings:
* AHPRA registration as a Registered Nurse
* Bachelor of Nursing (or equivalent)
* Minimum 3 years post-graduate clinical experience in a healthcare setting
* Excellent communication skills with an empathetic, patient-centred approach
* Strong time management, critical thinking, and decision-making abilities
* Confidence to work both independently and as part of a multidisciplinary team
* Understanding of evidence-based practice and care coordination principles
* Knowledge of health fund models and discharge planning systems (desirable)
