Job Title: Senior Case Management Professional
About the Role
The Integrated Case Management Caseworker provides high-quality professional casework services to young people subject to a youth justice intervention. As a key member of the team, you will be responsible for delivering best practice statutory casework services with minimal supervision.
Key Responsibilities
1. Cultural Capability: Demonstrate cultural capability and/or a commitment to developing cultural capability to ensure an inclusive culture that aligns with the Human Rights Act 2019 and supports departmental programs and community partnerships aimed at preserving cultural connections for Aboriginal and Torres Strait Islander peoples.
2. Statutory Casework Services: Deliver best practice statutory casework services to young people using high-level assessment and intervention skills to deliver integrated and culturally appropriate services.
3. Preparation and Presentation: Prepare and present information and evidence to the Children's Court, including conducting pre and post-court interviews, liaising with key court stakeholders, and preparing pre-sentence reports and affidavits as required.
4. Stakeholder Engagement: Engage with and establish partnerships with a broad range of internal and external stakeholders and service providers to establish an appropriate therapeutic alliance to achieve quality casework outcomes.
5. Case Planning: Formulate a singular case plan with consistent action-oriented goals to support the young person and their family members. This may include assisting parents/guardians, siblings, and/or young people to obtain employment, re-engage in education, address substance misuse, access physical and mental health services, access safe housing, learn responsible parenting skills, address anti-social behavior, and improve community participation.
6. Record and Information Management: Undertake a range of record and information management tasks, including maintenance of case records, preparation of reports, and data entry to ensure casework and accountability in accordance with policy and procedures.
7. Complex Case Management: Liaise with and seek advice and approval from relevant Youth Justice Service Centre staff in relation to the management of complex cases, development of programs, exercising of statutory delegations, payment of program-related costs, and other matters as required.
8. Training and Development: Actively participate in and deliver training and professional development activities to develop, maintain, and enhance knowledge and competencies to ensure service delivery is based on contemporary, evidence-based, best practice principles and is consistent with current practice trends and departmental objectives.
