Colac Area Health - Emergency Recovery Support Program Practitioner. Up to 3 x Full Time Positions - Short term Contract

About the role
Colac Area Health is seeking a team of people to deliver the Emergency Recovery Support Program up until the end of June 2026. The Emergency Recovery Support Workers will provide person-centred, trauma-informed support to individuals, families and communities impacted by emergency events (e.g. bushfire, flood, storm, public health emergency), in alignment with Victorian Government Emergency Management and Recovery principles. The role supports immediate relief needs and medium to long-term recovery planning, promotes community resilience, and ensures services are delivered in accordance with funding guidelines, professional practice standards, and legislative requirements.
Primary Responsibilities
* Deliver case support, case coordination and case management services to clients who have completed intake and assessment and been deemed eligible.
* Deliver services within the allocated service hour cap as informed via intake and assessment process
* Develop and implement individualised recovery plans addressing complex recovery support needs resulting from a major emergency in Victoria.
As our ideal candidate you will have:
* Relevant tertiary qualification in Social Work, Community Services, Psychology, Human Services or related discipline (or equivalent demonstrated experience).
* Demonstrated experience delivering structured case support, case coordination and/or case management for complex clients within community or human services settings.
* Ability to support client capability building and participation in recovery and good understanding of the impacts of major emergencies (e.g. bushfire, flood, storm, public health emergency) on individuals and families.
* Demonstrated application of trauma-informed, strengths-based and recovery-oriented practice frameworks.
* Demonstrated understanding of, or ability to rapidly acquire knowledge of:
* Emergency management resources across state, federal and local agencies
* Emergency Recovery Support Program (ERSP) Guidelines
* DFFH Service Agreement requirements
* Quality Care Close to Home
* Privacy and information sharing legislation
* Understanding of the Victorian community services system and referral pathways.
* Demonstrated ability to coordinate services across government and non-government agencies utilising strong interpersonal and communication skills to engage clients and stakeholders effectively.
* Experience accurately recording service delivery data and audit ready case files, in client management systems.
* Demonstrated understanding of intersectional and community-based recovery approaches and a commitment to culturally safe practice, including working respectfully with Aboriginal communities, culturally and linguistically diverse communities, people with disability, and other priority cohorts.
* Previous experience in emergency relief or disaster recovery programs
* Current WWCC and National Police Check
* Current Victorian Drivers Licence
