Job Description
The purpose of this role is to provide engaging, recreational and educational activities and programs that cater to the diverse needs of residents.
* Organize and participate in therapeutic groups and/or educational programs to promote resident well-being.
* Contribute to the assessment process by gathering information to develop individual care plans.
* Evaluate the effectiveness of therapy care plans and provide feedback to healthcare professionals.
* Collaborate with residents, their families and caregivers to create personalized programs.
* Work effectively with a multidisciplinary team to deliver integrated person-centered care.
* Liaise with service providers and advocate for residents and their families.
* Comply with guidelines and statistical data collection requirements.
