Key Responsibilities:
* Accurately identify changes in clients' needs using clinical procedures and AWACCS instruments.
* Evaluate patterns identified in clients to ensure effective management of client programs and policies.
* Lead and manage a team of Support Workers to ensure clients and business needs are met.
* Ensure WHS compliance and maintain a safe work environment.
* Effectively manage incidents and feedback in a timely manner.
* Support the development of recruitment strategies aligned with client requirements.
* Produce high-quality reports and assessments relevant to participants' care needs.

Requirements:
* Diploma in nursing (equivalent qualification highly desirable).
* Strong understanding of My Aged Care Home Care Packages.
* Exceptional communication and person-centred decision-making skills.
* Strong supervision and leadership abilities.
* High-level critical thinking and time management.
* NDIS Workers Clearance Check.
* DCSI/DHS Working with Children Check.
* Current driver's license.
* Current Police Check.
