About the Role
As a Case Manager, you will be responsible for delivering person‑centred and enablement‑focused case management to our clients. The role involves care coordination, administrative tasks, and rostering of client services. You will manage a caseload under CHSP/HCP approved funding, maintain budgets, onboard new clients, and ensure ongoing care plans support client independence. Building positive relationships with local teams and the wider organisation to improve outcomes for our clients is essential.
About You
- Experience in delivering quality, client‑focused case management
- Understanding of various funding sources available in the aged care sector
- Tertiary qualification in Community Services, Case Management or related discipline
- High level of computer systems proficiency, including data entry
- Experience developing care plans for complex clients
- Current and valid Australian driver’s licence
- Reliable, comprehensively insured vehicle
- Current First Aid qualifications (or willingness to obtain before commencement)
